Job Description:

A recognized healthcare network in New York is currently seeking an experienced, licensed Registered Nurse (RN) to join their staff in Rockland County as their new -Assistant -Director of Nursing . In this role, the Assistant -Director of Nursing will be responsible for leading a team of professionals coordinating care and directly providing bedside care.

Responsibilities:
The Assistant -Director of Nursing will:

• 
  
• Directly supervise -admission department, intake department and hospital liaison team
  
• Round -to the hospitals to ensure liaisons are fully equipped to execute role
  
• Maintain -the admission schedule and maximizes productivity of team
  
• Track -daily, weekly, and monthly metrics to ensure productivity standards are met
  
• Follow -up on all referrals that do not come onto service
  
• Ensure -department team members documents in the patient's clinical record when required; demonstrates proficiency with documentation requirements
  
• Actively collaborate -regarding QAPI activities and provides feedback and input into Performance Improvement
  
• Participate -in the analysis and reports on quality metrics
  
• Participate -in care delivery for patients as needed including conducting admissions, recertification visits, routine visits, and discharge visits
  
• Manage -the care for a caseload of patients if needed to support ebb and flow of census changes
  
• Participate -in after-hours shifts as needed including primary and back-up on-call
  
• Perform -other duties, as assigned
  
  
  Qualifications:
  
  
  • 
    
  • 4+ years of of Nursing experience
    
  • Graduate of an accredited School of Nursing
    
  • NYS Registered Nurse (RN) license
    
  • Experience in a Leadership and/or Supervisory role
    
  • Working knowledge of Healthcare software
    
  • Experience with Electronic Medical Record systems
    
  • Solid assessment and documentations kills

  • Great interpersonal skills
    
  • Excellent communication skills (written and verbal)
    
  • Strong attention to detail
    
  • Highly organized
    
    Desired Skills:
    
    
    • 
      
    • Bachelor's Degree in Nursing
      
    • Experience in a Home Health, Hospice, and/or Nursing Facility setting
