WCS Healthcare Partners is seeking a local contract nurse RN Case Management for a local contract nursing job in Ramsey, New Jersey.Job Description & Requirements

• Employment Type: Local ContractOur client is seeking a dedicated and compassionate Case Manager to manage the day-to-day operations and support services for those in need. This role is ideal for someone passionate about non-profit work and aligned with the mission of helping vulnerable individuals achieve stability and independence. This is a temp to perm role.
  
  -Job Function:
  
  
  • Meet with newly admitted residents to discuss goals, policies, and procedures.
    
    
  • Provide direct case management services, including referrals and issue resolution for mothers in shelter.
    
    
  • Assist residents in completing government assistance applications within the first two weeks of acceptance.
    
    
  • Collaborate with external partners in financial, housing, employment, childcare, and justice systems.
    
    
  • Identify and coordinate enrichment programs, such as parenting classes, life skills, and educational opportunities.
    
    
  • Work as part of a treatment team, coordinating services with staff and external agencies.
    
    
  • Assist clients in securing housing, employment, and other necessary resources. -
    
    
    
    Job Requirements:
    
    
    • Bachelor's degree in Social Work, Human Services, or related field.
      
      
    • 3-5 years of experience in case management or social work, preferably in a non-profit setting.
      
      
    • Knowledge of non-profit policies, procedures, and best practices.
      
      
    • Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint).
