Job Description:
Job Description Job Description Position Summary Our program is
a high-volume program. Team members take on separate roles and
responsibilities for the same clients to help them complete our
program as fast as possible. The person in this position will
provide direct services to clients by identifying client needs and
providing safe sleep education and cribs to clients, as well as
resources and referrals. A Cribs Advocate II will lead and
facilitate multiply weekly community workshops in-person and
virtually; and attend outreach activities to raise awareness about
SUID and safe sleep. This position will require data entry to
document client activities and requires frequent lifting of
20-40lbs. Essential Functions Responds to requests for cribs from
the community by conducting telephone intake and scheduling clients
for community workshops and home visits. Contact participants by
telephone, text, email, or home visits to register them for
workshops and provide resources as needed. Conducts client
assessments to determine areas of need. Learns about SUID,
breastfeeding, and other relevant topics and community resources.
Leads 3 to 6 community education workshops monthly on safe sleep
virtually and at sites throughout Philadelphia and surrounding
area. Assists in preparing for community workshops and home visits,
which can include reminders calls, emails, and text as well as
loading cribs and materials for deliveries and workshops. Delivery
cribs to clients and other community organizations. Conducts home
visits with families to deliver and set-up crib and provide safe
sleep/SIDS risk reduction education. Conducts health promotion
activities by providing client education, disseminating educational
literature, and distributing health materials which could include
condoms, feminine products, and vaccination information. Keep
careful records of all contacts made and follow-ups, as necessary.
Keeps accurate records of services received by clients. Ensures
confidentiality of client records, sharing pertinent information
only with the client’s approval. Routinely uses MCC client database
and other computer systems including e-mail, Internet, and word
processing. Attends orientation and ongoing training sessions on
maternal and child health and welfare, child abuse, housing,
nutrition, substance abuse, counseling, parenting, infant, and
family support issues. Other Tasks Refers participants needing more
intensive follow-up to appropriate agencies. Communicate with
supervisor and team regularly to report on and discuss work.
Participates in outreach efforts to locate and communicate with
pregnant women and caregivers. Assists in organizing client
meetings and other special events that celebrate mothers, fathers,
and caregivers. This could include community baby showers, chats
and celebrations created by MCC or other community organizations
partners. Attends community meetings, health fairs, baby showers
and represents MCC as requested by supervisor. Conducts follow-up
calls with Cribs for Kids clients to help rate the level or service
and education they received. Adheres to the MCC’s security
guidelines and ensures the appropriate handling of sensitive
information. Attends relevant staff meetings to promote
communication and execution of goals. Trains new Advocates or
students as requested. Supervises students and volunteers. Assumes
duties of supervisor as necessary and appropriate. Completes
special projects specific to the function of the department or as
needed for the department as directed by the supervisor. Other
duties as assigned within the scope of position expectations.
Knowledge, Skills, and Abilities Ability to organize tasks in an
efficient manner and follow-up and follow-through with strong
attention to detail in a high-volume department. Ability to operate
a computer and use a variety of common software programs including
Microsoft Office, spreadsheets, and customized databases. Effective
and accurate documentation of case notes and client data according
to established client management documentation procedures.
Understanding of MCC’s mission, goals, and objectives and ability
to work independently with an elevated level of energy and
contribute as part of a larger team. Knowledge of the concepts and
practices associated with community health issues, particularly
with pregnant women, prenatal care, parenting and other services
for mothers, young children, and their families. Ability to travel
in and around the Greater Philadelphia region to conduct workshops,
drop-offs, home visits and community outreach. Knowledge of and
sensitivity to issues facing the community served. Knowledge of
basic client management procedures for determining eligibility,
assessing needs, identifying resources, making referrals, following
up, and documenting client interactions. Strong people skills as
demonstrated by compassionate, courteous, cordial, cooperative, and
professional interaction with diverse groups of co-workers,
community partners, clients, and members of the community. Ability
to work 2 to 4 evenings per month and occasional weekends.
Experience, Education, and Licensure High School Diploma or
equivalent required. 1 year experience providing direct services in
a community outreach setting, or customer service preferred.
Experience with community health issues. Experience working with
maternal and child health issues preferred. Bilingual (Spanish,
Arabic, Haitian Creole) not required, but a plus. Valid drivers’
license required. Other Position Factors The other position factors

virtual elements. May require working non-traditional hours based
on operational needs.
