NPORT Staffing is seeking a licensed Physical Therapist for a full-time, hybrid position in Middletown, NY. This role combines home care patient evaluations throughout the week with treatment services provided in a skilled nursing facility rehabilitation gym. The ideal candidate is a motivated clinician who enjoys working across settings, managing a varied caseload, and delivering high-quality, patient-centered care. Key Responsibilities: Conduct physical therapy evaluations for multiple home care patients weekly Develop and implement individualized plans of care Provide skilled physical therapy treatment in a skilled nursing facility rehabilitation gym Treat patients with orthopedic, neurological, post-surgical, and geriatric conditions Document evaluations, daily notes, progress reports, and discharge summaries accurately and timely Collaborate with physicians and interdisciplinary healthcare teams Educate patients and caregivers on treatment plans, exercises, and safety Ensure compliance with Medicare guidelines, state regulations, and agency policies.
