Job Description:
At Carnegie Post Acute Skilled Nursing Facility, we are

a dedicated Licensed Practical Nurse (LPN) to join our team. This
role involves providing essential nursing care, supporting patient
rehabilitation, and aiding in the management of patient health
according to our high standards of care. The ideal candidate will
have proven experience in a skilled nursing facility, a
compassionate approach, and a strong commitment to patient-centric
healthcare. Administer nursing care to ill, injured, or disabled
patients within the facility Monitor patient health and record
signs Administer prescribed medications and treatments Collaborate
with registered nurses to administer care plans Assist in the daily
living activities of patients Educate patients and their families
on health maintenance and disease prevention Strong interpersonal
and communication skills Proficiency in maintaining accurate
patient records Ability to administer medication accurately and
safely Experience in a post-acute or rehabilitation environment
Empathetic and compassionate patient care Medical Insurance Dental
Insurance Vision Insurance Must possess a current, valid LPN
license in the state of employment CPR certified
