Description Atlantic Health is Seeking a Diabetes Nurse Practitioner in Pompton Plains, New Jersey Atlantic Health, one of New Jersey's largest non-profit healthcare networks, is seeking a Diabetes Nurse Practitioner (NP) to support Endocrine Services. The NP will support both outpatient services in Pompton Plains, as well as inpatient services at Chilton Medical Center. As a Diabetes NP, you will have a crucial role in delivering specialized diabetes care to adult patients aged 18 and above. Your responsibilities will include providing comprehensive diabetes care to patients, ensuring high-quality, evidence-based practice. Collaborating with a multidisciplinary team, you will develop and implement individualized treatment plans, educate, and support patients in self-management of their conditions, and contribute to the growth and advancement of the Endocrine program through active participation in quality improvement initiatives.
