Description Atlantic Health is Seeking a Diabetes Nurse
Practitioner in Pompton Plains, New Jersey Atlantic Health, one of
New Jersey's largest non-profit healthcare networks, is seeking a
Diabetes Nurse Practitioner (NP) to support Endocrine Services. The
NP will support both outpatient services in Pompton Plains, as well
as inpatient services at Chilton Medical Center. As a Diabetes NP,
you will have a crucial role in delivering specialized diabetes
care to adult patients aged 18 and above. Your responsibilities
will include providing comprehensive diabetes care to patients,
ensuring high-quality, evidence-based practice. Collaborating with
a multidisciplinary team, you will develop and implement
individualized treatment plans, educate, and support patients in
self-management of their conditions, and contribute to the growth
and advancement of the Endocrine program through active
participation in quality improvement initiatives. Additionally, you
will engage in continuous learning and professional development to

from accredited NP program Master’s degree required Certified
Diabetes Counselor Education Specialist (CDCES), or ability to
obtain a certification within 1 year of hire ACLS and BLS
certification required 2-3 years of clinical experience preferred,
ideally in an acute care or physician practice setting managing
patients with diabetes; new graduates will be considered Must have
excellent communication and computer skills Strong work ethic and a
team player who can work independently
