Description Registered Nurse Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses or disabilities. Monitors patients, administers medications, keep records, consult healthcare providers, educates patients and more. Assess patient knowledge and provides education in group and individual settings for patients with Type 1, Type 2, Gestational Diabetes, and Pre-Diabetes. Provides education tailored to special learning needs. Coordinates patient care and adjustments in care plans in coordination with clinicians. Principal Accountabilities: (including but not limited to) Coordinates individualized patient and support system assessment, education, planning and implementation of care goals. Collaborates with the referring clinicians and interdisciplinary team to work toward optimal patient education outcomes and glycemic control. Maintains professional education standards required to be a CDCES in an ADA approved Diabetes Self- Management Education program. Engages in research and quality improvement activities. Other duties as assigned. Schedule: Per diem, Evenings Monday - Friday (2 shifts per month) Schedule may vary based on department needs Qualifications Required: RN education must be obtained from an accredited institution BLS required Certification as a CDCES (Certified Diabetes Care and Education Specialist) requested within 1 year of hiring Prior RN roles in acute care or ambulatory diabetes Minimum of 1-3 years Preferred: Epic experience preferred BSN preferred
